1. In Main.NewUserTemplate, go to "More topic actions" again, select "Add or replace form on this topic", and select your new user form

2. Edit Main.NewUserTemplate, click on the "Add form" button, and select your new user form - (if it does not show up, add UserForm to the WEBFORMS setting in Main.WebPreferences first)
